IFSP-Individualized Family Service Plan.

 

Assessment and program development includes assessment of strengths,  needs and identification of services appropriate for a child and their family.  Families input, priorities, concerns and the identification of the supports and services needed are all considered when an IFSP is written.

 

A Periodic review includes an evaluation once a year with the family receiving a review of the plan at 6-month intervals.

 

Promptness after assessment-An IFSP should be developed within a reasonable time after the assessment.  With parental consent, early intervention services can begin prior to the completion of the assessment.

 

Content of plan is in writing and should contain several statements including:

current levels including cognitive, physical, communication, social, emotional, and adaptive development. 

the family’s resources, priorities, and concerns relating to enhancing the development of the family’s infant or toddler with a disability is included.
 

the major outcomes expected as well as the criteria, procedures, and timelines used to determine the degree to which progress toward achieving the outcomes are being made.  Whether modifications or revisions of the outcomes or services are necessary should be determined at this time.  

 the specific early intervention services necessary including the frequency, intensity, and the method of delivering services.   

early intervention services shall appropriately be provided.  

 the projected dates for initiation of services and the anticipated duration of services.

the name of the case manager who will be responsible for the implementation of the plan and coordination with other agencies and persons.

your needs as a family and recommendations that will enable your child to be successful.

             

 

Parental consent

Written consent is required of parents to carryout any early intervention services.  If consent is not provided for each service available only the services with written permission will be implemented.
