Your Portfolio should include the following:

1. Name, Address, Phone Number, E-mail, Student ID
If you want your portfolio mailed back to you, include a SASE.
or you may pick it up during the next semester in Dr. Engel's office.

2. NCTM membership number and names of journals you subscribed to.

3. Names of additional books purchased and/or books/journals read in library to support on-line course..

4. Statement that you communicated with the instructor at least one time per week.

5. Summary of other communications with students in the class. WebBoard Activities should be summarized.

6. Three CMP reports. Title of each CMP and results of work.

7. Three new CPM's that you created at high school level.

8. Name of school where you did Practicum, and Practicum Journal. (See Practicum)

9. Six one-page papers. (See Projects for details.)

10. Summary of visited Web Sites. Which three were the best?

11. Unit Plan with individual lesson plans.

12. Evaluation of on-line course. What did you find hard? esay? useful?
