NUR3049C - Population Based Care: Rural Cultural

Health Promotion Project
(Constitutes 30% of Final Grade)


Much of the practice of community health nursing involves client education to promote health, prevent illness, enhance self care and minimize the effects of health problems.  Over the semester you will develop a hralth promotion project in the practice component of the course.  The aim of the project is that you actually conduct a health promotion activity with an individual in your practice setting. As you evolve your project through the stages of assessment, design, implementation and evaluation we will discuss these developments within the group.

Utilize the following guidelines to help you develop your health promotion activity.

      1.  Select one health promotion area from the list below:

             Nutrition: Promoting Healthy Eating
             Nutrition: Decreasing Risk of Obesity
             Chronic Disease: Diabetes - Recognition 
             Chronic Disease: Diabetes - Self-management
             Health Risk: Hypertension: Recognition
             Health Risk: Hypertension: Self-Management

              Thoroughly research the area you have selected. Start broadly, examining 
              epidemiology,  risk groups, demographic issues, health significance, disease process, 
              and recommended health promotion activities.  Focus the broad health promotion area you 
              have selected to make it more specific to your proposed audience.  Write 500 words about  your 
              topic incorporating the research you have done.  Make sure you reference using APA style.
                                                                    (TOPIC RESEARCH-  30 Points )

         2.   Identify a target audience for your health promotion activity drawn from your knowledge of the
               cultural aggregates within the rural practice setting  used for this course.     Write 500 words 
               documenting  your assessment of  topic related  learning needs, the cultural  environment and of 
               individuals within the aggregate group you have selected.        (ASSESSMENT STAGE - 20 Points)

        3.    Develop two or three specific outcome objectives that you expect alearner to accomplish as a
               result of your health promotion activity and discuss the learning domains and learning tasks involved 
               in the presentation. Create a table to present  the objectives, associated content,  teaching 
               strategies/materials and evaluation.  Prepare your teaching materials. 
                                                                                                (DESIGN  STAGE - 30 Points)

        4.    Implement your health promotion activity with at least one individual within the practice setting
               demonstrating evidence of  your preparation, organization, and ability to tailor it to the situation. 
               Write a reflective journal of about 500 words reviewing this process including what you felt were the
               strengths and weaknesses of your session.  Outline how you would revise your health promotion 
               activity and why you would make the proposed changes. 
                                                     (IMPLEMENTATION  AND EVALUATION -  20 Points)

                                                                             TOTAL = 100  Points

    You are required to submit all documentation associated with your health promotion activity.
    Include your work from all 4 areas of the guidelines presented above. Your approach
    should be scholarly and reflect appropriate synthesis of and reference to literature
    throughout each of the four required stages.   Include use of APA format for
    referencing. The submission date for presenting your entire and complete health promotion 
    activity is  12 Noon July 17, 2000.  Late submissions will be penalized (- 3 marks per day).
