: : : : : : i don't think is the nurse problem for the loss, to my opinion the upper management should preven that from day one, and set a specific charge guidelines for the nurse to follow.

: : : : : I suggest that the nurse be taken out of the process as much as possible.

: : : : Then who would do it

: : : Basically, the computer. When the patient comes into the E.R., the patient will recieve a charge for his particular diagnosis or DRG. This charge would be based on the average use of supplies used for patients in this particular diagnosis. If the patient uses more, then the hospital suffers a loss. If the patient uses less, then the hospital gains. The nurse job would be to use the appropriate amount of supplies, necessary and take care of the patient.

: : the computer has to be run by human authority

: Very true.

Don't you think there are two problems with this bundling theory? First, that payers may have trouble with not having line items for which they are paying (and are therefore sure the pt used all of the services involved), and second, that it is inequitable to have some people use more resources but pay the same as others?
